| DETAILS REQUIRED FOR
APPLICATIONS FOR: DATING CERTIFICATE ONLY 1. Details of make and model of machine including frame & engine numbers. 2. Photograph of complete machine. 3. Clear photographs, or rubbings, of both the vehicles frame & engine numbers. 4. NACC membership number. 5. Cheque payable to NACC for £7.00 (£10.00 for non-members) 6. S.A.E. Dating certificates for Cyclemotor attachments will cover the engine unit and a statement as to the host bicycle being of a suitable date of manufacture for the issue of an `age related` mark. N.B. your machine must be insured and M.O.T.`d under its frame number when applying to a L.V.R.O. for an `age related` registration mark. NB. The NACC reserve the right to examine any machine at the applicants expense should the need arise to do so before a certificate is issued. RETENTION OF ORIGINAL REGISTRATION MARKS. 1. Form V765. 2. Form V55/5. 3. Two photographs of the complete machine. 4. Clear photographs or rubbings of both the frame & engine numbers. 5. Original logbook, tax disc, or other supporting documentation. 6. NACC membership number. 7. Cheque payable to NACC for £12.50.( £17.50 for non-members) 8. S.A.E. if you require confirmation that the application has been verified. 9 Stamped but unaddressed envelope approximately 9x6. N.B. All photographs are non-returnable. All machines must be complete and original and may be subject to examination by the DVLA. The NACC also reserve the right to examine any machine at the applicants expense should the need arise to do so before verification of such an application. When posting V765 applications to me it is advisable that you make sure that the correct rate of postage stamps are affixed to the envelope.
